Karen Preston, 54, walked away from the Adult Home on February 11, 2007, where she lived due to her for severe schizophrenia.

She was found in the woods about a mile away.

Adult Home Resident Karen Preston walked in circles, fell repeatedly, lost her shoes and finally curled up under a tree.

She was found dead from hypothermia.

On February 23, 2008, about a year after Ms. Preston wandered away, Ella Maye, a resident of the same Adult Home wandered away.

Ms. Maye, 78, suffered from dementia and heart disease.

Ms. Maye was believed to be crawling back to the Home when she crumpled and died from a heart attack on someone’s front yard.

The Adult Home’s staff had simply failed to conduct the hourly bed check.

Two dead residents who did not need to die that way.
